Qualification has three separate layers, and confusing them is why so many people get contradictory answers. The first is the labeled population: Wegovy and Zepbound are indicated for adults with obesity, or with overweight plus at least one weight-related condition. The second is clinical suitability, which is about contraindications and your wider history. The third is insurance criteria, which are set by your plan and are frequently stricter than the label. You can qualify on one layer and not another.

Layer One: The Labeled Population

Wegovy injection is indicated, in combination with a reduced-calorie diet and increased physical activity, to reduce excess body weight and maintain weight reduction long term in adults and pediatric patients aged 12 years and older with obesity, and in adults with overweight in the presence of at least one weight-related comorbid condition.

Zepbound covers chronic weight management in adults with obesity or with overweight plus at least one weight-related condition, and separately covers moderate to severe obstructive sleep apnea in adults with obesity. Foundayo (orforglipron), approved April 1, 2026, uses the same structural population for chronic weight management.

The pivotal trials enrolled adults with a body mass index of 30 kg/m² or greater, or 27 kg/m² or greater with at least one weight-related comorbid condition such as treated or untreated dyslipidemia or hypertension. Wegovy is the product with a pediatric weight indication, from age 12; Wegovy tablets do not carry it.

Layer Two: Clinical Suitability

Labeled contraindications come first. Both semaglutide and tirzepatide products are contraindicated in patients with a personal or family history of medullary thyroid carcinoma or with Multiple Endocrine Neoplasia syndrome type 2, and in patients with serious hypersensitivity to the drug or its excipients. Both carry a boxed warning about thyroid C-cell tumors observed in rodents.

Then history that changes the calculus: pancreatitis, gallbladder disease, diabetic retinopathy, severe gastroparesis — semaglutide is not recommended in patients with severe gastroparesis — and current or planned pregnancy, since weight loss offers no benefit to a pregnant patient and may cause fetal harm.

Then the medication list. Concomitant insulin or an insulin secretagogue raises hypoglycemia risk, and both labels address the effect of delayed gastric emptying on concomitant oral drugs, with Wegovy specifically directing increased clinical or laboratory monitoring for narrow-therapeutic-index medications.

Layer Three: Payer Criteria

Prior authorization criteria commonly require measured height and weight with dates, a calculated body mass index consistent with the labeled population, diagnosis codes for weight-related conditions, documentation of a prior structured attempt at diet and physical activity, and often a trial of a preferred alternative agent first.

These criteria are set by the payer, not the FDA, and can be stricter than the label. Many employer plans go further and exclude anti-obesity medications as a benefit category entirely, which clinical documentation cannot overcome.

For Medicare drug coverage, federal regulation identifies agents used for anorexia, weight loss, or weight gain among categories that may be excluded. A product prescribed against a non-weight indication is assessed differently.

Diagnoses That Change the Picture

Established cardiovascular disease supports Wegovy's cardiovascular risk reduction indication in adults with either obesity or overweight, which is not a weight-reduction claim and sometimes sits outside an anti-obesity exclusion.

Moderate to severe obstructive sleep apnea in an adult with obesity is a Zepbound indication in its own right, supported by two 52-week randomised placebo-controlled studies in 469 adults.

Noncirrhotic MASH with moderate to advanced liver fibrosis is a Wegovy indication under accelerated approval. Type 2 diabetes opens a different set of products entirely — Ozempic, Mounjaro, Rybelsus and Ozempic tablets.

Who Is Usually Not a Candidate

Anyone with a labeled contraindication. Anyone pregnant or planning pregnancy in the near term. Anyone whose weight concern is being driven by something a clinician should address first — an untreated thyroid disorder, a contributing medication, or an eating disorder.

People below the labeled thresholds are also outside the approved population. Prescribing below them would be off-label, which is lawful where a clinician judges it appropriate but creates no coverage obligation and carries the full weight of that clinician's judgement.

And anyone who cannot arrange continuity. Randomised withdrawal trials consistently show substantial regain after discontinuation, so starting without a plan to continue is a predictable route back to the starting point.

How to Find Out Where You Stand

Bring measured height and weight over time rather than estimates, a full medication and supplement list, family history covering thyroid cancer and endocrine neoplasia, and every weight-related diagnosis you carry.

Ask which approved indication applies to you rather than naming a brand. That framing lets the clinician select the right product and surfaces indications you may not have realised were relevant.
