There is no single best GLP-1 for weight loss, and the trial record does not contain the head-to-head evidence that would settle it. What exists is a set of separately approved products with separately generated results: SURMOUNT-1 ran 72 weeks in 2,539 adults and reported mean weight changes of -15.0%, -19.5% and -20.9% for tirzepatide versus -3.1% with placebo; the pivotal 68-week semaglutide trial in the Wegovy label reported -14.9% versus -2.4%. Those are different trials in different populations over different durations, which is exactly why the honest answer is a comparison of features rather than a ranking.

The Approved Options, Listed Accurately

Wegovy is semaglutide, available as injection across six strengths — including a 7.2 mg Wegovy HD pen — and as tablets at 1.5, 4, 9 and 25 mg. Zepbound is tirzepatide, a dual glucose-dependent insulinotropic polypeptide and GLP-1 receptor agonist, approved for chronic weight management and for moderate to severe obstructive sleep apnea in adults with obesity.

Foundayo (orforglipron), approved April 1, 2026 under NDA 220934, is an oral GLP-1 receptor agonist supplied as tablets at 0.8, 2.5, 5.5, 9, 14.5 and 17.2 mg and indicated in combination with a reduced-calorie diet and increased physical activity for adults with obesity or overweight plus at least one weight-related comorbid condition. Ozempic, Rybelsus, Ozempic tablets and Mounjaro are type 2 diabetes products with no weight-management indication.

What the Numbers Do and Do Not Support

Cross-trial comparison is unreliable. SURMOUNT-1 ran 72 weeks in 2,539 adults with obesity or overweight, excluding diabetes. The pivotal semaglutide trial ran 68 weeks in 1,961 adults with a mean baseline weight of 105.3 kg and mean BMI of 37.9 kg/m². The 72-week semaglutide trial supporting the 7.2 mg maintenance dosage enrolled 1,407 adults with mean baseline weight of 113 kg and mean BMI of 39.9 kg/m².

Different durations, different entry criteria, different baseline weights and different eras of background care. Placing three percentages side by side and declaring a winner requires ignoring all of it. What can be said is that both molecules produce substantial mean reductions relative to placebo in the populations studied.

Route of Administration Is a Real Differentiator

This is where the options genuinely diverge. Wegovy offers weekly injection or a once-daily 25 mg tablet, with the label containing switching provisions in both directions. Foundayo is an oral tablet. Zepbound is injection only; there is no FDA-approved oral tirzepatide and no oral Zepbound.

The oral products carry administration conditions in their labeling that the injections do not — the semaglutide tablets are taken on an empty stomach in the morning with limited water, swallowed whole, with a waiting period before food, other beverages or other oral medications. Whether that fits a person's life is a legitimate input into product choice.

Additional Indications May Decide It

For some people the tiebreaker is not weight at all. Zepbound carries an obstructive sleep apnea indication in adults with obesity. Wegovy injection carries a cardiovascular risk reduction indication and, under accelerated approval, an indication for noncirrhotic MASH with moderate to advanced fibrosis. Wegovy tablets carry the cardiovascular indication but not the MASH or pediatric indications.

If a person has one of those conditions, the product whose label covers it has an advantage no weight-loss percentage can offset. This is the most practical way the "which is best" question actually gets resolved in clinic.

Tolerability Determines What You Can Stay On

Efficacy in a trial is measured among people who continued. The Wegovy label reports that adverse reactions led to permanent discontinuation in 6.8% of treated adults versus 3.2% on placebo in the pooled weight-reduction trials, most often for nausea, vomiting and diarrhea. In the 72-week trial of the higher dosage, 5.4% on 7.2 mg discontinued for adverse reactions versus 1% on placebo.

Since the STEP 1 trial extension found that participants regained 11.6 percentage points of lost weight in the year after treatment stopped, the product a person can actually continue matters more than the product with the largest trial number. All three weight-management labels also state that concomitant use with another GLP-1 receptor agonist is not recommended.
