"GLP-1 compound" is doing two very different jobs in the same phrase, and confusing them causes real problems. In one sense a compound is simply a chemical entity — semaglutide and tirzepatide are compounds that act on the glucagon-like peptide-1 receptor. In the other sense, "compounded" is a regulatory term describing a preparation made by a licensed pharmacy for an individual patient, which is not the same thing as an FDA-approved drug product. This article separates the two, because the first is about biology and the second is about law.
GLP-1 the Hormone Comes First
Glucagon-like peptide-1 is a hormone your gut releases in response to eating. The Ozempic prescribing information describes it as a physiological hormone with multiple actions on glucose, mediated by GLP-1 receptors. The Wegovy label adds the other half: GLP-1 is a physiological regulator of appetite and caloric intake, and its receptor is present in several brain regions involved in appetite regulation.
Native GLP-1 is broken down within minutes by the enzyme DPP-4, which is why the hormone itself was never a practical medication. Solving that durability problem is what produced the drug class.
The hormone is also not acting alone. Glucose-dependent insulinotropic polypeptide is the other major incretin released after eating, and the fact that a second incretin exists is what made dual-receptor drugs a plausible design rather than a theoretical one.
What Makes a GLP-1 Receptor Agonist
An agonist is a molecule that binds a receptor and switches it on. Semaglutide is a GLP-1 analogue with 94% sequence homology to human GLP-1 that selectively binds to and activates the GLP-1 receptor. The Ozempic label explains the engineering: the principal mechanism of protraction producing semaglutide's long half-life is albumin binding, which reduces renal clearance and protects the molecule from metabolic degradation, and the compound is additionally stabilized against DPP-4.
The practical result is a drug that survives long enough to be dosed once weekly instead of continuously. That is the single design decision that made this class usable outside a hospital.
Half-life also explains why these medications change slowly. The Ozempic label notes an elimination half-life of approximately one week, with semaglutide present in the circulation for about five weeks after the last dose and steady-state exposure reached after four to five weeks of once-weekly administration. Nothing about this class happens quickly in either direction.
What These Compounds Actually Do
Three effects show up consistently in the labeling. Semaglutide stimulates insulin secretion and reduces glucagon secretion in a glucose-dependent manner, meaning the insulin response scales with how high blood glucose actually is. It delays gastric emptying. And it decreases calorie intake through effects the Wegovy label attributes to appetite.
The Wegovy pharmacodynamics section also notes that semaglutide lowers body weight with greater fat mass loss than lean mass loss. That is a description of what was measured, not a promise about body composition for any individual.
Glucose dependence is the safety feature that distinguishes this class from insulin secretagogues that work regardless of blood sugar. It does not eliminate hypoglycemia risk entirely: the labeling notes that concomitant use with insulin or an insulin secretagogue may increase that risk, which is a prescriber-level consideration rather than something to manage on your own.
The Approved Products in This Class
In the United States the semaglutide products are Ozempic injection, Wegovy injection and tablets, and the Rybelsus and Ozempic tablets. Tirzepatide is sold as Mounjaro for glycemic control in type 2 diabetes and as Zepbound for chronic weight management and for moderate to severe obstructive sleep apnea. Each brand has its own application, its own trials and its own labeling.
Brand names are not interchangeable shorthand for a molecule. Mounjaro and Zepbound contain the same active ingredient and are approved for entirely different things, which is why insurers, pharmacies and prescribers treat them as separate products.
The oral landscape has broadened too. Wegovy tablets at 1.5, 4, 9 and 25 mg sit inside the Wegovy label with a 25 mg once-daily maintenance dosage for cardiovascular risk reduction and weight reduction in adults, while Rybelsus and Ozempic tablets serve the type 2 diabetes indications. There is no FDA-approved oral tirzepatide.
Dual Agonists Hit Two Receptors
Tirzepatide is described in its labeling as a glucose-dependent insulinotropic polypeptide receptor and GLP-1 receptor agonist — a single compound with affinity for two incretin receptors rather than one. Its labeling reports an elimination half-life of approximately five days, enabling once-weekly dosing.
In the 72-week SURMOUNT-1 trial of 2,539 adults with obesity or overweight and without diabetes, mean weight change was −15.0%, −19.5% and −20.9% across the three doses studied versus −3.1% with placebo. Different molecule, different receptor profile, different trial program — which is why cross-trial comparisons should be read carefully.
Direct comparisons are rarer than the internet suggests. Trials differ in duration, in whether participants had diabetes, in the intensity of the lifestyle program, and in how missing data were handled. Two numbers from two different studies are not a head-to-head result, and treating them as one is the most common analytical error in this space.
What "Compounded" Means at a Pharmacy
Compounding is the preparation of a medication by a licensed pharmacy pursuant to a prescription for an individual patient. Compounded preparations are not FDA-approved, and the agency does not review them for safety, effectiveness or quality before they reach a patient. Section 503A of the Federal Food, Drug, and Cosmetic Act governs state-licensed pharmacies and physicians doing this work.
The regulatory picture shifted sharply in 2025. The FDA declared the tirzepatide shortage resolved in December 2024 and the semaglutide shortage resolved in February 2025, then set dates after which compounders could no longer produce preparations that are essentially copies of the approved products — February 18, 2025 for 503A pharmacies on tirzepatide and April 22, 2025 for 503A pharmacies on semaglutide.
The phrase "essentially a copy" is the hinge. Compounding did not become illegal; producing duplicates of commercially available approved products under a shortage rationale did. Compounding that reflects a documented clinical need for an individual patient remains within the 503A framework, which is why licensed compounding continues after those dates.
Telling the Two Apart
An approved product has an FDA-reviewed label, a national drug code, and a manufacturer accountable for every lot. A compounded preparation has a pharmacy, a prescription and a patient name. Neither of those is inherently a scandal — compounding is a long-standing and legitimate part of pharmacy practice — but they are not the same category and should never be described as if they were.
The failure mode to watch is the third thing: material sold online with no prescriber and no pharmacy, often labeled for research use. The FDA has announced its intent to restrict GLP-1 active pharmaceutical ingredients destined for non-FDA-approved compounded drugs and to act against marketing that presents compounded products as equivalent to approved ones.
A practical test: can you name the prescriber, name the pharmacy, and see the state license? If any of those three answers is missing, you are not looking at compounding in the regulated sense of the word, whatever the website calls itself.
