A GLP-1 medication activates the receptor for glucagon-like peptide-1, a hormone your gut releases when you eat, and in doing so it changes three things at once: how much insulin you release relative to your blood sugar, how quickly food leaves your stomach, and how hungry you feel. Everything else this class is known for — weight reduction, cardiovascular risk reduction, kidney outcomes — follows from those mechanisms and from the trials that tested them. The Hormone Being Imitated
The Ozempic label describes GLP-1 as a physiological hormone with multiple actions on glucose, mediated by GLP-1 receptors. The Wegovy label adds that GLP-1 is a physiological regulator of appetite and caloric intake, and that the receptor is present in several brain regions involved in appetite regulation.
Native GLP-1 is degraded within minutes by the DPP-4 enzyme. Medications in this class are engineered around that problem: semaglutide is a GLP-1 analogue with 94% sequence homology to human GLP-1, protracted by albumin binding that reduces renal clearance and stabilized against DPP-4, giving an elimination half-life of approximately one week.
That long half-life shapes the whole treatment experience. The Ozempic label notes that steady-state exposure is reached after four to five weeks of once-weekly administration and that semaglutide remains in the circulation for about five weeks after the last dose. Nothing in this class starts quickly, and nothing stops quickly either.
Glucose Control That Depends on Glucose
The label describes the glycemic mechanism precisely: semaglutide stimulates insulin secretion and lowers glucagon secretion, both in a glucose-dependent manner. When blood glucose is high, insulin secretion is stimulated and glucagon is inhibited; when it is not, that pressure eases.
That glucose dependence is a meaningful design feature. It does not eliminate hypoglycemia risk — the labeling notes that concomitant use with insulin or an insulin secretagogue may increase it, and that a dose reduction of those agents may be necessary — but it behaves differently from drugs that push insulin regardless of blood sugar.
The measured effect is substantial. The Ozempic label reports that in patients with type 2 diabetes, semaglutide 1 mg produced reductions versus placebo of 29 mg/dL for fasting glucose, 74 mg/dL for two-hour postprandial glucose, and 30 mg/dL for mean 24-hour glucose concentration.
Slower Gastric Emptying
Both labels state that semaglutide delays gastric emptying. The Ozempic label characterizes it as a minor delay in the early postprandial phase contributing to glucose lowering; the Wegovy pharmacodynamics section states it plainly as a standalone finding.
This has a consequence beyond fullness. Both labels flag that delayed gastric emptying may affect the absorption of concomitantly administered oral medications, and the two labels differ in how strongly they say so — Wegovy's drug interactions section recommends considering increased clinical or laboratory monitoring when used with oral medications that have a narrow therapeutic index. That is a conversation for your prescriber and pharmacist.
Appetite and Caloric Intake
The Wegovy label's pharmacodynamics section is the clearest statement of what drives weight change: semaglutide decreases calorie intake, and the effects are likely mediated by affecting appetite. Animal studies cited in the label show semaglutide distributing to and activating neurons in brain regions involved in regulating food intake.
The same section notes that semaglutide lowers body weight with greater fat mass loss than lean mass loss. Notably, nothing in either label describes increased energy expenditure — these are not thermogenic drugs, and describing them as fat burners misstates the mechanism.
This is why every weight-related indication is written as an adjunct to a reduced-calorie diet and increased physical activity. In the pivotal trials participants received instruction for roughly a 500 kcal per day deficit and counseling toward a minimum of 150 minutes of activity per week, beginning with the first dose. The medication was never studied on its own.
Effects Beyond Weight and Blood Sugar
The outcome trials are where this class earned its broader indications. In the cardiovascular outcomes trial reported in the Wegovy label as Study 1 — published as SELECT — 17,604 adults with established cardiovascular disease and either obesity or overweight, without diabetes, were randomized to semaglutide 2.4 mg or placebo. The estimated hazard ratio for major adverse cardiovascular events was 0.80 with a 95% confidence interval of 0.72 to 0.90, over a median follow-up of 41.8 months.
Other indications rest on their own programs. Ozempic carries a kidney outcomes indication in adults with type 2 diabetes and chronic kidney disease. Wegovy carries an accelerated-approval indication in noncirrhotic metabolic dysfunction-associated steatohepatitis with moderate to advanced fibrosis. Zepbound is approved for moderate to severe obstructive sleep apnea in adults with obesity as well as for chronic weight management.
The pattern is consistent: every additional indication required its own trial. That is worth remembering when you read that a GLP-1 medication "helps with" some condition — the question is always whether a trial was designed to answer that question, or whether someone observed a secondary finding and extrapolated.
Dual Agonists Add a Second Receptor
Tirzepatide is described in its labeling as a glucose-dependent insulinotropic polypeptide receptor and GLP-1 receptor agonist — one molecule acting at two incretin receptors, with an elimination half-life of approximately five days. It is sold as Mounjaro for glycemic control in type 2 diabetes and as Zepbound for weight management and sleep apnea.
In the 72-week SURMOUNT-1 trial of 2,539 adults with obesity or overweight and without diabetes, mean weight change was −15.0%, −19.5% and −20.9% across the three doses studied versus −3.1% with placebo. Whether a second receptor target is advantageous for a particular person is a clinical question, not a general one.
What These Medications Do Not Do
They do not raise metabolic rate. They do not work independently of food intake — every weight-related indication is labeled as an adjunct to a reduced-calorie diet and increased physical activity. They do not suit everyone: the labels carry a boxed warning about thyroid C-cell tumors observed in rodents whose human relevance has not been determined, and contraindications including a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2.
And they are not a short course. The trials behind these labels ran 64 to 72 weeks or longer, and the indications are written as long-term therapy. What happens when treatment stops is a legitimate question to raise before treatment starts.
