Wegovy and Zepbound occupy the same shelf: both hold FDA approval for chronic weight management. What makes the matchup interesting is that they get there on different molecules. This comparison stays close to what regulators approved, what the trials measured, and how the two feel to take — before turning to the genetics they share.

The two weight-management labels

Wegovy is semaglutide (Novo Nordisk), a GLP-1 receptor agonist. Zepbound is tirzepatide (Eli Lilly), a GIP/GLP-1 dual agonist. Both are labeled for chronic weight management, which is why they are so often compared directly. Elsewhere in the class, liraglutide and dulaglutide are GLP-1-selective, and retatrutide is an investigational triple agonist not FDA-approved as of April 2026.

GLP-1 alone versus GIP and GLP-1 together

The mechanistic split is one receptor versus two. Semaglutide engages the GLP-1 receptor; tirzepatide engages both GIP and GLP-1 receptors. In practice, all of these agents slow gastric emptying, temper post-meal glucose, and heighten satiety — but the dual-agonist design of tirzepatide is the structural reason the two brands are studied and dosed on separate tracks.

What each label actually covers

Indication history clarifies the boundaries. Wegovy (semaglutide 2.4 mg) earned chronic weight management in 2021. Zepbound (tirzepatide) earned chronic weight management in 2023 and added obstructive sleep apnea in adults with obesity in 2024 through SURMOUNT-OSA. Their diabetes-labeled relatives are Ozempic (2017) and Mounjaro (2022); Rybelsus (2019), Trulicity (2014), and Victoza (2010) round out the class. Retatrutide is not approved.

Trial programs behind each brand

Semaglutide's weight evidence comes from STEP, with SUSTAIN and SELECT covering diabetes and cardiovascular outcomes. Tirzepatide's comes from SURMOUNT and SURPASS, plus SURMOUNT-OSA. The most-cited direct signal is SURPASS-2, which compared tirzepatide with semaglutide 1 mg in type 2 diabetes and reported greater mean weight reduction with tirzepatide in that specific trial. Because endpoints and populations differ, weight-management conclusions should be read per trial.

How tolerability compares

Tolerability looks broadly similar. Both brands carry the class boxed warning for thyroid C-cell tumor risk seen in rodent studies and are contraindicated in patients with a history of medullary thyroid carcinoma or MEN 2. Gastrointestinal symptoms lead the side-effect list and cluster around dose escalation. Serious but uncommon events include pancreatitis, gallbladder disease, and dehydration-linked kidney injury for either molecule.

Cost at U.S. list price

As of April 2026, list prices for the FDA-approved brands range roughly $815 to $1,620 per month according to manufacturer-published figures. Real-world spending depends on insurance, the indication written, savings-card eligibility, and whether a compounded alternative is available through a licensed pathway. List price and net cost are rarely the same number.

Shared genetic architecture

Different molecules, same underlying biology. FTO shapes appetite and adiposity set-point; GLP1R encodes the receptor both engage; MC4R anchors energy balance; and TCF7L2 links to glucose homeostasis and GLP-1 secretion. These pathway-level variants do not predict which brand suits you — they describe the metabolic terrain a clinician is prescribing into for either.

Are Wegovy and Zepbound both approved for weight management?

Yes. Wegovy (semaglutide 2.4 mg) received chronic weight-management approval in 2021, and Zepbound (tirzepatide) in 2023. Zepbound also gained an obstructive sleep apnea indication in 2024. They are the two branded weight-management options in this class, built on different molecules.

Does the SURPASS-2 trial mean Zepbound outperforms Wegovy?

Not as a blanket claim. SURPASS-2 compared tirzepatide with semaglutide 1 mg in type 2 diabetes and showed greater mean weight reduction with tirzepatide in that trial. Weight-management studies used different doses and endpoints, so the result should not be generalized across every setting.

Is one brand easier to tolerate than the other?

Their tolerability profiles are broadly comparable. Gastrointestinal effects predominate for both and are most noticeable during dose escalation. Both carry the same class boxed warning and the same contraindications. Individual tolerability varies, which is a conversation to have with a prescribing clinician.
