What the Human Evidence Supports
The 2026 review notes that dietary interventions including β-glucan supplementation, fermentable carbohydrate diets, probiotics, polyphenols and plant polysaccharides can influence GLP-1 secretion through microbiome-dependent mechanisms, drawing on both human and preclinical studies.
A companion review in Advances in Nutrition is franker about magnitude: weight loss with fibre alone is typically more modest than with GLP-1 receptor agonists and depends on the type, dose and duration of intake. It also notes that tolerability can be limited by bloating or gas, particularly if intake is increased too rapidly.
That is an honest summary of the state of the field. Fibre engages the same gut-brain axis through a physiological route, delivers broad benefits including gut barrier function and short-chain fatty acid enrichment, and produces smaller weight effects.

Two facts break that chain. First, endogenous secretion is subject to immediate enzymatic degradation, so raising release does not raise sustained exposure the way a degradation-resistant agonist does. Second, the pivotal pharmacotherapy trials measured weight over 68 to 72 weeks against placebo with structured lifestyle support in both arms; almost no supplement has been tested that way.
One specific caution: the widely cited randomised controlled trial reporting weight loss from apple cider vinegar was retracted in September 2025. It should not be cited, and any article still resting on it is out of date.
Sleep, Activity and the Rest of the Picture
Physical activity, sleep and alcohol intake all influence appetite regulation and metabolic health through routes that are only partly incretin-mediated. Improving them is worth doing on its own terms and does not need to be justified by a GLP-1 mechanism.
This matters for how you evaluate advice. A recommendation to eat more fibre, sleep adequately and move more is well supported by general evidence. Attaching a specific hormonal claim to it adds nothing and often signals that something is being sold.

Whether pharmacotherapy is appropriate is a clinical question about diagnosis, risk and labeled indications, and it belongs with a prescriber. The Advances in Nutrition review proposes a pragmatic framing that places dietary fibre and lifestyle measures as the foundation of care with pharmacotherapy reserved for higher-risk individuals — a sequencing decision, not a competition.
What the Approved Labels Imply About Diet
The prescribing information for these medications is itself a reminder that diet is never displaced. Every weight-management indication in the Wegovy label is written as applying "in combination with a reduced calorie diet and increased physical activity", and the Zepbound label uses the same construction. The diabetes labels use "as an adjunct to diet and exercise".
Those phrases are not decorative. They describe the conditions under which the trials were run and therefore the conditions under which the results were observed. In the Wegovy programme, participants received instruction for an approximately 500 kcal per day deficit and physical activity counselling recommending a minimum of 150 minutes per week, beginning with the first dose and continuing throughout.
So the choice was never between raising GLP-1 through food and taking a medication. The medication was studied on top of the dietary intervention, and the nutritional work described here remains relevant whether or not pharmacotherapy is ever part of the picture.
