In the pivotal obesity trial, the largest average weight reduction occurred at the highest dose studied — but "most effective" and "highest" are not the same claim, and the Zepbound prescribing information is built around that distinction. The label names 2.5 mg once weekly as the starting dosage for all indications, lists 5 mg, 10 mg and 15 mg as maintenance dosages for weight reduction, and caps the maximum at 15 mg once weekly. Which of those a given person ends up on is a clinical decision made by a prescriber over months, weighing response against tolerability. What the evidence can tell you is what each dose produced on average, and how much of the benefit was already captured before the ceiling.
What the Pivotal Trial Found, Dose by Dose
In SURMOUNT-1, 2,539 adults with a body mass index of 30 or more, or 27 or more with at least one weight-related complication, excluding diabetes, were randomized in equal proportions to once-weekly tirzepatide at 5 mg, 10 mg or 15 mg or to placebo for 72 weeks, including a 20-week dose-escalation period. Mean baseline body weight was 104.8 kg and mean BMI was 38.0.
Mean percentage change in weight at week 72 was −15.0% at 5 mg, −19.5% at 10 mg and −20.9% at 15 mg, against −3.1% with placebo. Those are the numbers behind the dose table, and they show a real but decelerating dose-response: the step from placebo to 5 mg was worth about 12 percentage points, the step from 5 mg to 10 mg about 4.5, and the step from 10 mg to 15 mg about 1.4.
Why the Ceiling Is Not Automatically the Target
That flattening curve is the whole reason the label lists three maintenance dosages instead of one. If 15 mg were simply better for everyone, there would be no clinical reason to name 5 mg and 10 mg as endpoints rather than as steps along the way.
The counterweight is tolerability. Gastrointestinal adverse reactions are the most commonly reported effects with tirzepatide, and their frequency rises with exposure. The label's escalation structure — 2.5 mg for four weeks, then 5 mg, then increases in 2.5 mg increments after at least four weeks at the current dose — exists to give a prescriber repeated decision points at which to stop, hold, or continue rather than to march every patient to the maximum.
The Maximum Tolerated Dose Concept
Later trials in the program made this explicit by randomizing participants to a maximum tolerated dose rather than a fixed one. In SURMOUNT-5, the 72-week head-to-head trial of tirzepatide versus semaglutide in adults with obesity and without type 2 diabetes, the tirzepatide arm was defined as 15 mg or the maximum tolerated dose, and the comparator arm as semaglutide 2.4 mg or its maximum tolerated dose.
That design choice reflects how these medications are actually used. The clinically relevant dose is the highest one a person can take consistently, which is not knowable at the outset and is not the same for everyone. A dose that causes enough nausea to prompt discontinuation delivers no weight reduction at all, and the trial designs acknowledge that directly.
What the Head-to-Head Comparison Showed
SURMOUNT-5 enrolled roughly 750 adults with obesity and without type 2 diabetes and ran 72 weeks. In a published subpopulation analysis of 383 of those participants, least-squares mean percentage change in body weight at week 72 was −20.1% with tirzepatide versus −12.9% with semaglutide, a difference of −7.2 percentage points. Reductions in waist circumference and BMI were also greater, and separated early — waist circumference from week 4 and BMI from week 12.
A separate post hoc analysis of 425 SURMOUNT-5 participants with prediabetes at baseline reported mean body weight reduction of −21.5% with tirzepatide versus −14.5% with semaglutide at week 72, alongside greater HbA1c reduction and a higher proportion reverting to normoglycemia. These are subgroup analyses rather than the primary endpoint, and they should be read as consistent with the main result rather than as standalone evidence.
Duration and Continuation Compete With Dose
Dose is not the only variable that determines outcome, and it may not be the most important one. In SURMOUNT-4, participants received open-label tirzepatide at a maximum tolerated dose of 10 mg or 15 mg for 36 weeks, then were randomized to continue or to switch to placebo for a further 52 weeks. Most of those switched to placebo regained weight over that period.
The implication is uncomfortable but important: reaching a higher dose for a few months matters less than remaining on an effective dose over years. Obesity behaves as a chronic, relapsing condition, and the withdrawal trials across both the semaglutide and tirzepatide programs point the same way. A dose someone sustains for three years will usually outperform a higher one they stop in month five.
Obstructive Sleep Apnea Has Its Own Range
Zepbound holds a second indication — moderate to severe obstructive sleep apnea in adults with obesity — and the label handles dosing differently for it. The starting dosage is the same 2.5 mg once weekly, but the maintenance dosages listed for that indication are 10 mg or 15 mg rather than the 5 mg, 10 mg and 15 mg range named for weight reduction.
That difference is a reminder that "most effective dose" is meaningless without specifying the endpoint. The dose that maximizes weight reduction and the dose studied for apnea-hypopnea index reduction were not established by the same trials, and the label reflects the evidence for each separately.
Who Decides, and On What Basis
No article, calculator, or test determines a dose. A prescriber does, using your diagnosis, your other medications and conditions, your response over successive four-week intervals, and how you tolerate each increase. The label supplies the boundaries; everything inside them is individualized judgment gathered over time.
