Every FDA-approved GLP-1 weight-management label uses the same phrase, and it is the most important sentence in the whole document: the medication is indicated in combination with a reduced-calorie diet and increased physical activity. That is not a disclaimer bolted on for legal cover. It describes the conditions under which the drug was studied, and therefore the only conditions under which its published results apply. The role of a GLP-1 medication in a weight-loss effort is to make a sustained calorie deficit biologically achievable for people for whom it previously was not — not to replace the deficit.
What "Adjunct" Means in Practice
In the pivotal trials, participants did not simply receive an injection and go home. In the Zepbound weight-reduction program, patients received instruction on a reduced-calorie diet of roughly a 500 kcal/day deficit and physical activity counseling with a recommended minimum of 150 minutes per week, beginning with the first dose and continuing throughout, plus counseling on behavior modification strategies. The placebo groups received the same instruction. The published differences between arms are therefore differences on top of a structured lifestyle intervention.
This is why the honest framing is a partnership rather than a substitution. The medication changes appetite signaling and how full you feel; the diet, the protein intake, the activity and the sleep still determine a great deal of the outcome, and they determine almost all of what happens to body composition rather than body weight alone.
What the Trials Actually Produced
The magnitude is genuinely large by the standards of previous obesity pharmacotherapy. In the Zepbound label's Study 1, a 72-week trial in 2,539 adults with obesity or overweight plus a weight-related condition and without type 2 diabetes, mean weight change from baseline was −15.0%, −19.5% and −20.9% at the 5 mg, 10 mg and 15 mg dosages, versus −3.1% with placebo. In the same study, 85.1%, 88.9% and 90.9% of treated patients lost at least 5% of body weight, compared with 34.5% on placebo.
For semaglutide, the corresponding 68-week weight-management trial reported roughly a −14.9% mean change versus −2.4% with placebo. The Wegovy label now also includes a higher-dose arm: in the study supporting the 7.2 mg once-weekly dosage, 72 weeks of treatment in 1,407 adults produced mean changes of −18.8% at 7.2 mg and −15.5% at 2.4 mg versus −3.9% with placebo.
The Part People Skip: What Happens When Treatment Stops
Two withdrawal trials answer this directly, and both point the same way. In STEP 4, 803 adults who had already lost a mean of 10.6% during a 20-week run-in were randomized either to continue semaglutide 2.4 mg or to switch to placebo. Over the following 48 weeks, the continued-treatment group lost a further 7.9% while the placebo group regained 6.9% — a 14.8 percentage-point difference.
The tirzepatide equivalent is Study 4 in the Zepbound label, an 88-week randomized withdrawal trial. Patients lost an average of 20.9% during a 36-week open-label lead-in, at which point mean body weight had fallen from 107.3 kg to 85.2 kg. They were then randomized to continue or to switch to placebo for 52 weeks, and the group that stopped regained weight. Obesity behaves like a chronic condition in these datasets, and the medication behaves like a chronic treatment.
Benefits That Are Not Measured on the Scale
Some of the strongest evidence in this class is not about weight at all. In SELECT, 17,604 adults aged 45 or older with pre-existing cardiovascular disease and a BMI of 27 or greater but without diabetes were randomized to semaglutide 2.4 mg or placebo. A primary cardiovascular endpoint event occurred in 6.5% of the semaglutide group versus 8.0% on placebo, a hazard ratio of 0.80 over a mean follow-up of just under 40 months.
Function matters too. In STEP-HFpEF, 529 patients with heart failure with preserved ejection fraction and a BMI of 30 or higher received semaglutide 2.4 mg or placebo for 52 weeks; the mean change in the Kansas City Cardiomyopathy Questionnaire clinical summary score was 16.6 points with semaglutide versus 8.7 with placebo, and the 6-minute walk distance improved by 21.5 m versus 1.2 m. These are the findings that drove Wegovy's cardiovascular indication and, separately, its liver indication.
The Costs the Brochures Underplay
Gastrointestinal effects are the dominant tolerability issue across the class. The most common adverse reactions in the Wegovy label, reported at 5% or more, include nausea, diarrhea, vomiting, constipation, abdominal pain, headache, fatigue, dyspepsia, dizziness, abdominal distension, eructation, flatulence, gastroesophageal reflux disease and hair loss. Severe gastrointestinal reactions were reported in 4.1% of Wegovy injection patients versus 0.9% on placebo in the adult weight-reduction trials.
There are also warnings that require monitoring rather than reassurance: acute pancreatitis, gallbladder disease with a specific note that rapid weight loss itself raises the risk of cholelithiasis, acute kidney injury following dehydration from vomiting or diarrhea, resting heart rate increases, and rare postmarketing reports of pulmonary aspiration during general anesthesia or deep sedation. None of these are reasons not to treat; all of them are reasons the treatment belongs inside a clinical relationship.
What Stays Yours
Appetite suppression creates a window, and what you put into that window is what you keep. Reduced intake without adequate protein and resistance training tends to cost lean mass alongside fat mass, which is a poor trade for long-term metabolic health. Sleep, alcohol, stress and activity all continue to influence the result, and none of them is affected by the injection.
The medication also cannot tell you when it is working badly. That judgment — whether side effects are acceptable, whether progress is reasonable, whether something else is going on — requires a clinician following you over time. The role of a GLP-1 in a weight-loss journey is best understood as one instrument in a longer clinical relationship, not the whole of it.
