GLP-1 medications help type 2 diabetes by doing what the body's own glucagon-like peptide-1 hormone does, only for far longer. According to the Ozempic prescribing information, semaglutide is a GLP-1 analogue with 94% sequence homology to human GLP-1 that selectively binds and activates the GLP-1 receptor, stimulating insulin secretion and lowering glucagon secretion — both in a glucose-dependent manner. It also produces a minor delay in early postprandial gastric emptying, which slows the rate at which glucose from a meal appears in the circulation. The downstream result is lower fasting and after-meal glucose, and in the large outcome trials, measurable reductions in cardiovascular and kidney events.
What "Glucose-Dependent" Means, and Why It Matters
That phrase is the single most important detail in the mechanism. Native GLP-1 is an incretin hormone released from the gut in response to food, and the insulin response it triggers scales with how much glucose is actually present. When blood glucose is high, insulin secretion is stimulated and glucagon secretion is inhibited. When glucose is not elevated, the signal largely stands down.
This is why GLP-1 receptor agonists behave differently from older agents that drive insulin release regardless of glucose level. The Ozempic label notes that patients receiving it in combination with an insulin secretagogue such as a sulfonylurea, or with insulin, may have an increased risk of hypoglycemia including severe hypoglycemia, and that reducing the dose of the concomitant medication may be necessary. In that scenario the hypoglycemia risk is contributed largely by the partner drug.
The long duration of action is engineered separately from the mechanism. The label attributes semaglutide's half-life of approximately one week to albumin binding, which decreases renal clearance and protects the peptide from metabolic degradation, together with a structural modification that stabilizes it against the DPP-4 enzyme.
What the Glycemic Numbers Actually Look Like
In a 30-week double-blind monotherapy trial of 388 adults with type 2 diabetes inadequately controlled with diet and exercise, the label reports HbA1c changes of −1.4% and −1.6% for the two semaglutide dosages studied versus −0.1% for placebo, from mean baselines near 8.1%. Seventy-three percent and 70% of those treatment groups reached an HbA1c below 7%, against 28% on placebo.
Against an active comparator the margin narrows but persists. In a 30-week trial versus insulin glargine in patients already taking metformin or metformin with a sulfonylurea, HbA1c fell 1.2% and 1.5% on the two semaglutide dosages versus 0.9% on glargine, and 55% and 66% of patients reached below 7% versus 40%.
Pharmacodynamic data in the same label quantify the day-to-day effect. After 12 weeks of treatment at steady state, patients with type 2 diabetes showed reductions versus placebo of 29 mg/dL for fasting glucose, 74 mg/dL for two-hour postprandial glucose, and 30 mg/dL for mean 24-hour glucose concentration.
Weight Change Travels With It — But That Is Not the Indication
Body weight fell in those same diabetes trials. Compared with sitagliptin over 56 weeks, mean changes from baseline were −4.2 kg and −5.5 kg versus −1.7 kg. Compared with insulin glargine over 30 weeks, mean changes were −3.2 kg and −4.7 kg while the glargine arm gained 0.9 kg.
Those are secondary observations inside programs designed around glycemic endpoints. Ozempic's approved indications are glycemic control in adults with type 2 diabetes, reduction of the risk of major adverse cardiovascular events in adults with type 2 diabetes and established cardiovascular disease, and reduction of the risk of sustained eGFR decline, end-stage kidney disease and cardiovascular death in adults with type 2 diabetes and chronic kidney disease. There is no weight-management indication in that label and no BMI criterion anywhere in it. Wegovy is the semaglutide product approved for weight reduction.
The Cardiovascular and Kidney Evidence
SUSTAIN 6 was a multi-center, multi-national, placebo-controlled, double-blind cardiovascular outcomes trial in 3,297 patients with inadequately controlled type 2 diabetes and atherosclerotic cardiovascular disease, followed for a minimum observation time of two years. The estimated hazard ratio for time to first major adverse cardiovascular event was 0.74 (95% CI 0.58 to 0.95). The trial was published in the New England Journal of Medicine in 2016.
The kidney evidence comes from FLOW, in which 3,533 patients with type 2 diabetes and chronic kidney disease were randomized and followed for a median of 41 months. Mean baseline eGFR was 47 mL/min/1.73m², and median baseline urine albumin-to-creatinine ratio was 568 mg/g. Semaglutide was superior to placebo on the composite of sustained eGFR decline of at least 50%, sustained eGFR below 15, chronic renal replacement therapy, renal death or cardiovascular death, with a hazard ratio of 0.76 (95% CI 0.66 to 0.88). All-cause death carried a hazard ratio of 0.80.
The label adds two honest caveats. The treatment benefit on the primary composite endpoint was not evident in patients already taking an SGLT2 inhibitor at baseline, though there were few events in that subgroup. And the mechanism of the kidney-related risk reduction has not been established.
The Risks the Label Requires Prescribers to Weigh
Ozempic carries a boxed warning for risk of thyroid C-cell tumors. In mice and rats, semaglutide caused a dose-dependent and treatment-duration-dependent increase in thyroid C-cell tumors at clinically relevant exposures; whether it does so in humans has not been determined. It is contraindicated in patients with a personal or family history of medullary thyroid carcinoma or with multiple endocrine neoplasia syndrome type 2, and in patients with a serious hypersensitivity reaction to semaglutide or the product's excipients.
The warnings section covers acute pancreatitis, diabetic retinopathy complications, acute kidney injury due to volume depletion, severe gastrointestinal adverse reactions, hypersensitivity reactions, acute gallbladder disease, and pulmonary aspiration during general anesthesia or deep sedation. The most common adverse reactions reported in at least 5% of treated patients are nausea, vomiting, diarrhea, abdominal pain and constipation.
An Adjunct, Not a Replacement
The indication language is precise: an adjunct to diet and exercise. GLP-1 therapy does not displace the rest of diabetes care — metformin, blood pressure and lipid management, glucose monitoring, retinal and kidney surveillance. It is layered onto that foundation, and the outcome trials that produced the numbers above were all run on top of standard of care.
One reassuring finding sits in the label: efficacy was not impacted by age, gender, race, ethnicity, baseline BMI, baseline body weight, diabetes duration, or level of renal function impairment. What varies most between individuals is tolerability and the interaction with the rest of a person's medication list, which is exactly the territory of a prescriber rather than an article.
Where Compounded Semaglutide Fits
Compounded semaglutide is a different regulatory category from the approved brands. It is not an FDA-approved finished product, the FDA does not review compounded drugs for safety, effectiveness or quality before they are marketed, and it is prepared by a licensed pharmacy pursuant to a prescription for an individual patient.
