The most common adverse reactions to Ozempic injection are gastrointestinal. Section 6.1 of the Ozempic prescribing information states that the most common adverse reactions reported in at least 5% of patients treated with Ozempic are nausea, vomiting, diarrhea, abdominal pain, and constipation. Those five account for the overwhelming majority of what people experience, they are dose-related, and the label reports that most of them clustered in a specific window of treatment. Understanding which effects are common and expected — and which are the rarer ones the label warns about — is the difference between an informed conversation with a prescriber and an anxious one.
What the Trial Data Actually Show
The label's placebo-controlled trial table gives incidence figures at the two studied maintenance strengths against placebo. Nausea occurred in 15.8% of patients at 0.5 mg and 20.3% at 1 mg, versus 6.1% with placebo. Vomiting occurred in 5% and 9.2%, versus 2.3%. Diarrhea occurred in 8.5% and 8.8%, versus 1.9%. Abdominal pain occurred in 7.3% and 5.7%, versus 4.6%. Constipation occurred in 5% and 3.1%, versus 1.5%.
Two things stand out. The placebo columns are not zero — a meaningful fraction of people report these symptoms on no active drug at all, which is worth remembering before attributing every symptom to a medication. And the gradient with strength is real for nausea and vomiting, which is the clinical basis for the label's stepwise approach to increasing strength rather than starting at a maintenance level.
Timing: Most of It Happens During Escalation
The label is specific about when these reactions occur: the majority of reports of nausea, vomiting, and/or diarrhea occurred during dose escalation. That is the period when circulating drug concentration is rising, and it is why the first weeks at any new strength are the ones prescribers ask about most closely.
The corresponding discontinuation figures give a sense of scale. More patients receiving Ozempic 0.5 mg (3.1%) and 1 mg (3.8%) discontinued treatment due to gastrointestinal adverse reactions than patients receiving placebo (0.4%). So while roughly one in five people reported nausea at the higher strength, only a small minority stopped therapy because of gastrointestinal effects.
Why the Gut Is Where This Shows Up
Semaglutide is a GLP-1 analogue with 94% sequence homology to human GLP-1, and it acts by selectively binding to and activating the GLP-1 receptor. Among the mechanisms the label describes for its effect on blood glucose is a minor delay in gastric emptying in the early postprandial phase. GLP-1 receptors are distributed in tissues involved in digestion and appetite regulation, so gastrointestinal effects are not an off-target surprise — they are downstream of the same signalling that produces the intended effect.
This also explains why these effects tend to be most pronounced when exposure changes rather than at any fixed level. The system adapts. What it adapts to is stability, which is part of why the label emphasizes consistent once-weekly administration.
The Warnings That Are Not Common but Are Serious
Ozempic carries a boxed warning for risk of thyroid C-cell tumors. In rodents, semaglutide causes dose-dependent and treatment-duration-dependent thyroid C-cell tumors at clinically relevant exposures; the label states it is unknown whether Ozempic causes thyroid C-cell tumors, including medullary thyroid carcinoma, in humans, because the human relevance of the rodent finding has not been determined. It is contraindicated in patients with a personal or family history of medullary thyroid carcinoma or with Multiple Endocrine Neoplasia syndrome type 2.
Beyond the boxed warning, section 5 covers acute pancreatitis, diabetic retinopathy complications, hypoglycemia with concomitant use of insulin secretagogues or insulin, acute kidney injury due to volume depletion, severe gastrointestinal adverse reactions, hypersensitivity reactions, acute gallbladder disease, and pulmonary aspiration during general anesthesia or deep sedation. Section 5.4 separately warns never to share a pen between patients, even with a changed needle.
How the Common and the Serious Connect
Acute kidney injury due to volume depletion is the clearest example of the link. Persistent vomiting or diarrhea causes fluid loss, and fluid loss is the pathway to the renal warning. That is why a prescriber's response to "I have been vomiting for three days" is different from their response to "I felt queasy on Tuesday" — the concern is not the nausea itself but where sustained gastrointestinal losses lead.
The same logic applies to hypoglycemia. Ozempic on its own is not the primary driver, but combined with insulin or an insulin secretagogue the label flags the risk explicitly. Anything that changes food intake — which gastrointestinal side effects certainly do — interacts with that combination. These are prescriber decisions, not self-management decisions.
What to Report and When
Severe or persistent abdominal pain, particularly radiating to the back and with or without vomiting, is the presentation the pancreatitis warning is written around. Vision changes matter because of the retinopathy warning. Signs of a hypersensitivity reaction, a neck mass, difficulty swallowing, shortness of breath, or persistent hoarseness are all in the label's counselling points.
One practical item is easy to miss: because of the pulmonary aspiration warning, anyone scheduled for a procedure involving general anesthesia or deep sedation should tell the surgical and anesthesia teams they are taking a GLP-1 medication, well in advance. Any strategy for managing common side effects belongs with your prescriber, who can weigh it against your full clinical picture.
Compounded Semaglutide and the Side-Effect Question
Compounded semaglutide contains the same active molecule as the FDA-approved semaglutide products, including the injection and the oral semaglutide tablets, so the pharmacology behind these effects is the same. What differs is regulatory status: compounded preparations are not FDA-approved finished products, and the FDA does not review them for safety, effectiveness, or quality before marketing. Incidence figures describe populations, not individuals. A 20.3% nausea rate at the higher studied strength means roughly four in five participants did not report nausea, and the 6.1% placebo rate means some of what was reported would have occurred without the drug. Neither number tells any particular person what they will experience.
The trial data also come from a defined setting: adults with type 2 diabetes, in monitored studies, at studied strengths, over defined periods. Real-world use varies on all of those axes. That is an argument for tracking your own experience — what, when, how severe, and at what point in treatment — and bringing that record to your prescriber, rather than trying to match yourself against a table.
