There is no Ozempic dosage you should choose, because choosing is not the patient's job — the FDA-approved label defines a fixed set of dosages, and a prescriber selects within it based on your glycemic response, your tolerability, and which of the label's indications applies to you. That answer is less satisfying than a number, but it is the accurate one, and understanding how the label is built explains why the number you see online is almost never the number that belongs to you. The Label Sets a Range, Not a Recommendation for You
Ozempic is administered once weekly. The prescribing information describes an initiation dosage of 0.25 mg once weekly for four weeks, followed by an increase to 0.5 mg once weekly, with maintenance dosages of 0.5 mg, 1 mg, or 2 mg selected on the basis of glycemic control. The maximum recommended dosage is 2 mg once weekly. Those are the only strengths the approved product is labeled for.
The label is explicit that the opening step exists for a reason unrelated to blood sugar: the escalation is there to reduce the risk of gastrointestinal adverse reactions. That framing matters. The first weeks are not a low-effectiveness period to be rushed through; they are a tolerability period built into the approval.
Why the Label Escalates at All
Warnings and Precautions section 5.7 addresses severe gastrointestinal adverse reactions, and the label states Ozempic is not recommended in patients with severe gastroparesis. The most common adverse reactions reported in at least 5% of treated patients are nausea, vomiting, diarrhea, abdominal pain, and constipation. Section 5.6 links volume depletion from those reactions to postmarketing reports of acute kidney injury, some requiring hemodialysis, with the label directing that renal function be monitored during initiation and escalation.
Read together, those sections explain the structure. A stepwise increase is not bureaucratic caution; it is the mechanism by which the approved product manages a real and documented risk. Compressing it is the specific behavior the FDA has flagged as harmful, which is covered further below.
A Higher Dosage Is Not Automatically a Better One
The label contains its own dose-comparison data. In a 40-week double-blind trial of 961 adults with type 2 diabetes on metformin with or without a sulfonylurea, Ozempic 2 mg was compared directly with Ozempic 1 mg. HbA1c fell by 2.1% on 2 mg versus 1.9% on 1 mg — a difference of 0.2%, with a 95% confidence interval of −0.31 to −0.04. Mean body weight change was −6.4 kg on 2 mg and −5.6 kg on 1 mg, and the label reports that the difference between arms in body weight change at week 40 was not statistically significant.
That is a modest incremental glycemic benefit and no demonstrated weight advantage, in a trial specifically designed to answer the question. It is a useful corrective to the assumption that more milligrams is simply more result. It is also why the label frames maintenance dosage as a decision based on whether additional glycemic control is needed, rather than as a ladder everyone is expected to climb.
What Determines Where a Person Lands
The indication does some of the work. For the type 2 diabetes and chronic kidney disease indication, the label describes a maintenance dosage of 1 mg once weekly. For glycemic control, maintenance may be 0.5 mg, 1 mg, or 2 mg. So two people can be on the same medication at different dosages for entirely legitimate reasons that have nothing to do with how well either is doing.
Individual factors do the rest. The label contraindicates Ozempic in anyone with a personal or family history of medullary thyroid carcinoma or with Multiple Endocrine Neoplasia syndrome type 2, and in anyone with a serious hypersensitivity reaction to semaglutide or the product's excipients. It flags increased hypoglycemia risk when Ozempic is used with insulin or an insulin secretagogue such as a sulfonylurea, and notes that reducing the dose of those medications may be necessary. It also notes that Ozempic delays gastric emptying and may affect absorption of concomitant oral medications. None of that is resolvable from a blog post.
Time Does More Work Than Milligrams
Semaglutide's pharmacokinetics explain much of the perceived plateau people describe in early weeks. Absolute bioavailability is 89%, maximum concentration is reached one to three days after a dose, and steady-state exposure is achieved only after four to five weeks of once-weekly administration. The elimination half-life is approximately one week, and the label notes semaglutide will be present in the circulation for about five weeks after the last dose.
Exposure increases in a dose-proportional manner across 0.5 mg, 1 mg, and 2 mg — the label reports mean steady-state concentrations of roughly 65 ng/mL and 123 ng/mL for 0.5 mg and 1 mg. The practical implication is that a given dosage has not finished doing what it does until roughly a month has passed. Judging a dosage before then is judging an incomplete picture.
The Presentations Shape the Conversation Too
Dosage is not only a number; it is also a physical product. The Ozempic label describes three single-patient-use pens, each tied to particular strengths: 2 mg per 3 mL, delivering four doses of 0.25 mg and two doses of 0.5 mg or four doses of 0.5 mg; 4 mg per 3 mL, delivering four doses of 1 mg; and 8 mg per 3 mL, delivering four doses of 2 mg. The 05/2026 revision also lists single-dose prefilled syringes at 0.25 mg, 0.5 mg, and 1 mg per 0.5 mL, supplied four to a carton.
That structure is why a dosage change is not simply a dial adjustment. Moving between maintenance dosages generally means a different presentation, a new prescription, and sometimes a different coverage determination. It is one more reason the question routes through a prescriber and a pharmacy rather than through a chart. It is also a contrast worth noting with compounded preparations, where concentrations are not standardized across pharmacies at all.

The FDA has documented what happens when dosing is improvised.
