Wegovy is not a single dose — it is a ladder, and the strength you start on is deliberately not a treatment dose. Why the Dose Escalates at All

Escalation is a tolerability strategy, not a way to make the drug work better in stages. Semaglutide slows gastric emptying and acts on appetite-regulating signaling, and the body adapts to those effects over a period of weeks. Introduce the full effect immediately and nausea, vomiting, and diarrhea are far more likely; introduce it gradually and the same endpoint dose is reached with fewer people abandoning treatment along the way.

This is why the trial programs reported gastrointestinal events as mostly mild to moderate and concentrated during the escalation period rather than spread evenly across treatment. It is also why the label frames the low strengths explicitly as starting and escalation dosages rather than as maintenance options. A dose that is only meant to be passed through is not a dose anyone is expected to stay on.

The Maintenance Dosage Is the Destination

For weight reduction, the label identifies 2.4 mg once weekly as the usual recommended maintenance dosage for Wegovy injection, with 1.7 mg once weekly available as an alternative. Escalation proceeds in steps at roughly monthly intervals, and the label instructs prescribers on how to handle patients who do not tolerate a step — which is precisely the decision that has to sit with a clinician who knows your history rather than with a schedule copied from the internet.

The number of steps is why people describe getting to a maintenance dose as taking months rather than weeks. That is the design working as intended. It also means the first months are not a fair test of the medication's effect, which matters if you are trying to judge early on whether it is working for you.

Wegovy HD and the 7.2 mg Addition

The ceiling moved in 2026. FDA approved Wegovy HD, semaglutide 7.2 mg once weekly, on March 19, 2026, and the strength now appears in the label's dosage forms alongside the others. The label conditions its use narrowly: it applies to patients who have tolerated the 2.4 mg dosage for at least four weeks and for whom additional weight reduction is clinically indicated.

The supporting evidence came from the STEP UP trial, which ran 72 weeks in 1,407 adults with a body mass index of 30 kg/m² or greater and without diabetes, randomized 5:1:1 to 7.2 mg, 2.4 mg, or placebo alongside lifestyle intervention. Mean weight reduction was approximately 20.7% at 7.2 mg and 17.5% at 2.4 mg among those who remained on treatment, against roughly 2.4% with placebo. Analyzed regardless of adherence, the corresponding figures were about 18.8%, 15.5%, and 3.9%. A higher ceiling is not automatically the right answer for any individual — it is an option the label makes available under specific conditions.

Wegovy Tablets Are a Different Dose Structure

Wegovy tablets are taken orally once daily rather than injected weekly, and their maintenance dosage is 25 mg once daily. They escalate in steps at roughly monthly intervals through the 1.5 mg, 4 mg, and 9 mg strengths before reaching maintenance. Milligrams are not comparable across the two forms, because oral semaglutide is absorbed very differently from injected semaglutide — a 25 mg tablet is not ten times a 2.4 mg injection in any meaningful sense.

The two forms also do not carry identical indications. According to the FDA-posted Wegovy labeling, Wegovy tablets are indicated to reduce the risk of major adverse cardiovascular events in adults with established cardiovascular disease and either obesity or overweight, and to reduce excess body weight and maintain weight reduction long term in adults with obesity or with overweight plus at least one weight-related comorbid condition. The injection carries those uses plus two more that the tablets do not: pediatric patients aged 12 years and older with obesity, and noncirrhotic metabolic dysfunction-associated steatohepatitis with moderate to advanced liver fibrosis.

The Indication Can Shape the Dose Target

Because Wegovy injection carries several indications, the maintenance dosage is not stated once and left there. For cardiovascular risk reduction the label identifies 2.4 mg once weekly as recommended with 1.7 mg as an alternative; for MASH it identifies 2.4 mg once weekly, with provision to decrease to 1.7 mg if needed. Someone treated for weight and someone treated for liver fibrosis are working from the same label but not necessarily toward the same target, and the reason for treatment is part of what determines where the ladder ends.

One Rule That Applies to Everyone

The label states that concomitant use of Wegovy tablets or Wegovy injection with other semaglutide-containing products, or with any other GLP-1 receptor agonist, is not recommended. This is the mistake most likely to arise when someone is transitioning between products, holds a leftover supply, or is treated by two clinicians who are not talking to each other.

It is also worth noting that a compounded semaglutide preparation is not Wegovy and does not come with the Wegovy label. Compounded products are not FDA-approved finished products, and whatever a compounding pharmacy dispenses will carry its own pharmacy labeling — which is the document that governs, along with the prescriber who wrote the order.

Where Your Actual Schedule Comes From

Nothing on a website can tell you which strength you should be on, when to move, or what to do if a step does not agree with you. Those decisions depend on how you have responded, what other conditions you have, and what your prescriber is treating. The label defines the range; the clinician applies it to you.
