Switching from Mounjaro to Ozempic is a prescriber-managed change, and there is no published milligram-for-milligram conversion between the two — the amount of tirzepatide you were taking does not translate into an equivalent amount of semaglutide. Both are once-weekly injectable medications approved for type 2 diabetes, and that shared indication is the honest starting point for this question. The Mounjaro prescribing information indicates it as an adjunct to diet and exercise to improve glycemic control in adults and pediatric patients 10 years of age and older with type 2 diabetes mellitus. The Ozempic prescribing information carries three indications, all of which also require type 2 diabetes. Neither product is approved for weight management.
What Each Product Is Approved to Do
Mounjaro has a single indication: glycemic control in type 2 diabetes, in adults and in pediatric patients aged 10 and older. Its labeled maximum is 15 mg once weekly in adults and 10 mg once weekly in pediatric patients, and it is supplied in 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg, and 15 mg presentations.
Ozempic covers more regulatory ground within the same disease. Alongside glycemic control, it is indicated to reduce the risk of major adverse cardiovascular events — cardiovascular death, non-fatal myocardial infarction, or non-fatal stroke — in adults with type 2 diabetes and established cardiovascular disease, and to reduce the risk of sustained eGFR decline, end-stage kidney disease, and cardiovascular death in adults with type 2 diabetes and chronic kidney disease. Its labeled maximum dosage is 2 mg once weekly.
Why People Change
The most common reasons are not clinical failure. Formulary changes, prior authorization denials, pharmacy supply, and out-of-pocket cost move more people between these products than efficacy does. When a plan drops one and covers the other, the switch is a coverage event that happens to have clinical consequences.
Some reasons are clinical. Those two additional Ozempic indications matter for someone with established cardiovascular disease or chronic kidney disease alongside type 2 diabetes, because they represent outcomes the FDA reviewed evidence for rather than inferred. Tolerability is the other genuine driver: gastrointestinal adverse reactions are common across this class, and an individual may do better on one agent than another for reasons that are not predictable in advance.
The Two Drugs Do Not Work the Same Way
Tirzepatide is a dual glucose-dependent insulinotropic polypeptide and GLP-1 receptor agonist. Semaglutide is a selective GLP-1 receptor agonist. Moving between them is not like switching between two versions of the same drug — you are removing activity at one receptor system entirely.
That difference is why the change tends to be felt, particularly by people who had reached a high tirzepatide dose. It also explains why the transition is a clinical decision rather than an administrative one, since glycemic control, other diabetes medications, and any insulin or insulin secretagogue in the regimen all interact with the change.
There Is No Dose Conversion Table
Neither label contains a conversion between tirzepatide and semaglutide, and none exists in the approved labeling for either product. Ozempic's initiation dosage is described in its label as a starting dosage for treatment initiation, not as a maintenance dosage and not as something selected on the basis of what a patient took previously. Its ceiling is 2 mg once weekly regardless of where someone was on the tirzepatide scale.
The practical implication is that anyone quoting you an equivalence — a specific tirzepatide dose "equals" a specific semaglutide dose — is offering something the evidence does not support. How a prescriber sequences a transition is their judgment, informed by your glycemic control, your other medications, your tolerance history, and your monitoring. It is not a lookup.
What the Head-to-Head Trial Actually Showed
These two molecules were compared directly in SURPASS-2, published in the New England Journal of Medicine. The open-label 40-week trial randomized 1,879 patients with type 2 diabetes to tirzepatide 5 mg, 10 mg, or 15 mg, or to semaglutide 1 mg. Mean baseline glycated hemoglobin was 8.28% and mean weight 93.7 kg.
Estimated mean reductions in glycated hemoglobin were 2.01, 2.24, and 2.30 percentage points across the three tirzepatide doses versus 1.86 percentage points with semaglutide, and tirzepatide was noninferior and superior at all three doses. Reductions in body weight were also greater with tirzepatide, with least-squares mean estimated treatment differences of −1.9 kg, −3.6 kg, and −5.5 kg. Two caveats matter when reading that as a reason not to switch: the comparator was semaglutide 1 mg rather than the 2 mg maximum now on the Ozempic label, and group averages do not predict what any one person experiences.
The Rule Everyone Should Know Before Switching
Do not overlap the two. Labeling across this class advises against concomitant use with other GLP-1 receptor agonists, and stacking a residual supply of one medication onto a newly started one is the most predictable way to turn a routine transition into a serious adverse event. If you have leftover product, tell the prescriber rather than working it into the schedule.
The same caution applies to compounded preparations. A compounded semaglutide or tirzepatide product is not an FDA-approved finished product and is not a generic version of either brand, and it should be part of the same disclosure to whoever is managing the change.
What the Conversation Should Cover
Bring the specifics: which product and strength you are on now, how long you have been on it, what your recent glycated hemoglobin looked like, what side effects you have had, what else you take — especially insulin or an insulin secretagogue, since hypoglycemia risk is a labeled concern with semaglutide in that combination — and what triggered the question, whether that is coverage, cost, supply, or how you feel.
Ask what monitoring is planned around the change and when you should expect to be reassessed. And treat the pharmacy label on whatever you are dispensed as the authority on the product in your hand. That document, and the clinician who wrote the prescription, are the only sources that can tell you how your transition should run.
