Ozempic is not approved for weight loss, and that is the first thing worth getting straight before discussing dosage or mechanism. The Ozempic prescribing information lists three indications, all of them in type 2 diabetes: improving glycemic control as an adjunct to diet and exercise, reducing the risk of major adverse cardiovascular events in adults with established cardiovascular disease, and reducing the risk of sustained eGFR decline, end-stage kidney disease and cardiovascular death in adults with chronic kidney disease. Weight loss appears nowhere in that list. The same molecule, semaglutide, does hold a weight-management approval — under the brand name Wegovy, at different dosages, supported by different trials. Understanding why the distinction exists explains most of what people actually want to know.
How Semaglutide Works
Semaglutide is a GLP-1 analogue with 94% sequence homology to human GLP-1 that acts as a GLP-1 receptor agonist, selectively binding to and activating the receptor targeted by the native hormone. GLP-1 is a physiological hormone with multiple actions on glucose, mediated by those receptors.
The label describes the glucose-lowering mechanism in specific terms: semaglutide stimulates insulin secretion and lowers glucagon secretion, both in a glucose-dependent manner. When blood glucose is high, insulin secretion is stimulated and glucagon secretion is inhibited. The mechanism also involves a minor delay in gastric emptying in the early postprandial phase. In the pharmacodynamics section, the label states directly that semaglutide lowers fasting and postprandial blood glucose and reduces body weight.
Its durability comes from chemistry rather than from a different receptor. The principal mechanism of protraction is albumin binding, which decreases renal clearance and protects the molecule from metabolic degradation, and semaglutide is further stabilized against breakdown by the DPP-4 enzyme. The result is an elimination half-life of approximately one week, with steady-state exposure reached after 4 to 5 weeks of once-weekly administration.
The Dosages Ozempic Actually Comes In
Ozempic is initiated at 0.25 mg subcutaneously once weekly for 4 weeks, a starting dosage intended to reduce the risk of gastrointestinal adverse reactions rather than to control glucose. After 4 weeks it increases to 0.5 mg once weekly. If additional glycemic control is needed after at least 4 weeks on 0.5 mg, the dosage may increase to 1 mg once weekly, and after at least 4 weeks on 1 mg it may increase to 2 mg once weekly.
The recommended maintenance dosage is 0.5 mg, 1 mg, or 2 mg once weekly based on glycemic control, and the maximum recommended dosage is 2 mg once weekly. That ceiling matters to this conversation: the semaglutide dosages studied and approved for chronic weight management are higher than anything Ozempic's label permits. These are not the same regimen wearing two names.
How Much Weight Changed in the Ozempic Trials
Body weight was measured throughout the diabetes program as a secondary outcome, and the results are in the label. In a 30-week placebo-controlled monotherapy trial, mean changes from baseline were −1.2 kg with placebo, −3.8 kg with Ozempic 0.5 mg and −4.7 kg with Ozempic 1 mg. In a 56-week trial against sitagliptin in patients also taking metformin and/or a thiazolidinedione, mean changes at week 56 were −4.2 kg with 0.5 mg, −5.5 kg with 1 mg and −1.7 kg with sitagliptin.
It is worth noting what these trials were not. Participants had type 2 diabetes, were frequently taking other glucose-lowering medications, and were followed for 30 to 56 weeks against a glycemic endpoint. Weight was recorded, analyzed and reported, but no aspect of the trial design was optimized to produce or detect maximal weight change, and dosages stopped at 1 mg in both of the trials described above.
Those are real reductions, and they are also considerably smaller than the figures people associate with semaglutide for obesity. The reason is straightforward: different dosages, different populations, different trial durations, and glycemic rather than weight-based primary endpoints.
What the Weight-Management Evidence Actually Looks Like
The obesity program was designed differently from the ground up. STEP 1, published in the New England Journal of Medicine in 2021, randomized adults with overweight or obesity to once-weekly semaglutide 2.4 mg or placebo for 68 weeks and reported a mean change in body weight of −14.9% with semaglutide versus −2.4% with placebo.
Those results support Wegovy, not Ozempic. Per the Wegovy prescribing information, the recommended maintenance dosage of Wegovy injection for weight reduction in adults is either 1.7 mg or 2.4 mg once weekly, and for patients who tolerate 2.4 mg for at least 4 weeks with additional weight reduction clinically indicated, the dosage may be increased to a maximum of 7.2 mg once weekly. Ozempic's label tops out at 2 mg. The gap is not a technicality.
Considerations That Belong in the Conversation
Semaglutide products carry a boxed warning regarding the risk of thyroid C-cell tumors. In rodents, semaglutide causes dose-dependent and treatment-duration-dependent thyroid C-cell tumors at clinically relevant exposures; whether it causes thyroid C-cell tumors including medullary thyroid carcinoma in humans has not been determined. Ozempic is contraindicated in patients with a personal or family history of medullary thyroid carcinoma or with Multiple Endocrine Neoplasia syndrome type 2, and in patients with a serious hypersensitivity reaction to semaglutide or any of its excipients.
The labeled warnings and precautions include acute pancreatitis, diabetic retinopathy complications, hypoglycemia when combined with insulin or an insulin secretagogue, acute kidney injury due to volume depletion, severe gastrointestinal adverse reactions, hypersensitivity reactions, acute gallbladder disease, and pulmonary aspiration during general anesthesia or deep sedation. The most common adverse reactions reported in at least 5% of treated patients are nausea, vomiting, diarrhea, abdominal pain and constipation.
There is also a reproductive consideration that gets overlooked: the label directs that semaglutide be discontinued in women at least 2 months before a planned pregnancy because of its long washout period. None of this is a reason to avoid the medication or to take it. It is the material a prescriber weighs against your specific history, and it is why these drugs are prescription-only.
Off-Label Use, and Where Compounded Semaglutide Sits
A licensed clinician may prescribe an approved medication outside its labeled indication when they judge it clinically appropriate. That is lawful and routine across medicine. It does not change the approval status, does not obligate an insurer to cover it, and does not mean the agency reviewed evidence for that purpose.
Compounded semaglutide is a different category entirely. The FDA states that unapproved versions do not undergo its review for safety, effectiveness and quality before they are marketed, that compounded drugs are not FDA approved, and that patients should obtain a prescription from their doctor and fill it at a state-licensed pharmacy. Ozempic and Wegovy are the FDA-approved semaglutide injections; a compounded preparation is not a generic of either, because neither has an approved generic.
