Some people can eat aged cheese, drink red wine, and finish a bowl of leftovers with no trouble. Others get a pounding headache, a flushed face, or a runny nose within an hour. One under-recognized reason for that difference sits in a single gene: AOC1, which builds the enzyme that clears histamine from your gut.

What the AOC1 gene does

AOC1 (amine oxidase copper-containing 1) encodes diamine oxidase, or DAO. DAO is the primary enzyme that breaks down histamine you take in from food and histamine released in the digestive tract. It is produced mainly by cells lining the small intestine, where it acts as a frontline gatekeeper before dietary histamine reaches the bloodstream.

When DAO activity is high, ingested histamine is degraded efficiently and rarely causes symptoms. When DAO activity is low, histamine can accumulate faster than the body clears it — a state often described as histamine intolerance. The AOC1 gene is one of the main reasons DAO capacity varies so much from person to person.

How DAO clears histamine in the gut

Histamine is a signaling molecule involved in immune responses, stomach acid production, and nerve communication. Your body makes it, but you also absorb it from fermented and aged foods — cured meats, aged cheeses, wine, sauerkraut, and certain fish. DAO oxidizes histamine into an inactive compound so it does not circulate and trigger receptors elsewhere.

This is a capacity system, not an on-off switch. If dietary histamine load exceeds available DAO, the excess spills over. That is why the same glass of wine bothers a person on one day and not another — total histamine exposure, gut health, and enzyme supply all shift the balance.

AOC1 variants linked to lower DAO activity

Several named single-nucleotide polymorphisms in AOC1 have been associated with reduced DAO enzyme activity. The most studied is rs10156191 (a Thr16Met change), where the minor allele has been linked to lower serum DAO in multiple cohorts. Two others — rs1049742 and rs1049793 — have also been associated with decreased DAO function, and carrying more than one variant appears to compound the effect.

These variants do not guarantee symptoms. Many carriers have no histamine problem at all, because DAO output is also shaped by intestinal health, medications, hormones, and diet. A genotype describes a predisposition in one pathway; it is not a diagnosis of histamine intolerance.

Histamine, migraines, and the DAO connection

Histamine is a known migraine trigger. It can dilate blood vessels and activate trigeminal nerve pathways involved in headache. Research has reported that a meaningful share of people with migraine also show low DAO activity, and some studies observed AOC1 variants more frequently in migraine groups. The proposed link is straightforward: less DAO means more circulating histamine, and more histamine means a lower threshold for a histamine-sensitive headache.

The evidence is associative, not deterministic. Migraine is multifactorial — hormones, sleep, stress, and other genes all contribute. But for people whose headaches track with high-histamine foods, the AOC1–DAO axis is a biologically plausible piece of the picture worth discussing with a provider.

Beyond AOC1: the second histamine pathway

DAO is not the only way the body handles histamine. Inside cells, a separate enzyme called HNMT (histamine N-methyltransferase) breaks histamine down, and it dominates in tissues like the brain and airways. So histamine clearance depends on two genetic pathways working together. Someone with reduced capacity in both may be more sensitive than someone with a variant in just one.

This is why looking at a single gene in isolation can be misleading. Histamine biology is a network, and your personal tolerance reflects the combined setting of several variants plus everything in your environment.
