What the Current Label Lists

The label also lists single-dose syringes at 0.25, 0.5, 1, 1.7, and 2.4 mg, and a single-patient-use Wegovy FlexTouch pen containing 9.6 mg per 3 mL that delivers four 2.4 mg doses.

Wegovy tablets are listed at 1.5, 4, 9, and 25 mg. The recommended maintenance dosage of the tablets is 25 mg once daily for cardiovascular risk reduction and weight reduction in adults. That is a genuinely different product from the injection, with a narrower set of indications.

The Escalation Schedule Behind the Numbers

Wegovy injection is initiated at 0.25 mg once weekly for four weeks and then escalated every four weeks according to the labeled schedule to reach the maintenance dosage. The usual recommended maintenance dosage of the injection is 2.4 mg once weekly, and the label directs readers to the full prescribing information for maintenance dosages by indication.

The tablets follow their own schedule: initiated at 1.5 mg once daily for 30 days, then titrated every 30 days toward the maintenance dosage. Where any individual should be on either schedule is a prescriber decision made on response and tolerability, not a target to chase.

What the 7.2 mg Data Actually Showed

Study 8 in the label was a 72-week trial that enrolled 1,407 patients with obesity, excluding type 2 diabetes, randomized 5:1:1 to Wegovy 7.2 mg, Wegovy 2.4 mg, or placebo once weekly. Mean baseline body weight was 113 kg and mean BMI was 39.9.

At week 72, mean change in body weight was −18.8 percent at 7.2 mg, −15.5 percent at 2.4 mg, and −3.9 percent with placebo. The difference between 7.2 mg and 2.4 mg was −3.3 percent, with a 95 percent confidence interval of −4.9 to −1.6. Responder rates were 88.7 percent achieving at least 5 percent weight loss at 7.2 mg and 83.7 percent achieving at least 10 percent.

Study 9, in 512 patients with obesity and type 2 diabetes, reported −13.2 percent at 7.2 mg, −10.4 percent at 2.4 mg, and −3.8 percent with placebo over 72 weeks. The incremental benefit of the higher dosage was real but modest in both trials.

Higher Is Not Automatically Better

The incremental 3.3 percent between 2.4 mg and 7.2 mg in Study 8 came with a tolerability cost: 5.4 percent of patients on 7.2 mg permanently discontinued because of adverse reactions versus 1 percent on placebo. In Study 9 the figure was 5.5 percent versus 2 percent.

This is the shape of most dose-response relationships in this class. Benefit rises less than proportionally while adverse reactions rise steadily, which is why labels specify a "usual recommended" maintenance dosage rather than defaulting everyone to the maximum. The right dosage is the one that a specific person tolerates and responds to, determined by their prescriber.

The FlexTouch Pen Is a Meaningful Addition

The current label includes a single-patient-use Wegovy FlexTouch pen — 9.6 mg per 3 mL, delivering four 2.4 mg doses — with its own warnings and precautions subsection titled "Never Share WEGOVY FlexTouch Between Patients." That subsection was added in a 2026 revision.

The statement that Wegovy comes only in single-dose pens was accurate before that revision and is no longer correct. It is a useful reminder that labeling changes, and that reference sites lag the manufacturer's own prescribing information. When a detail matters, check the current PI.

Where the "And Beyond" Question Leads

The label states plainly that concomitant use of Wegovy tablets or Wegovy injection with other semaglutide-containing products or with any other GLP-1 receptor agonist is not recommended. Stacking products is not a route to a higher effective dose.
