MTHFR is one of the most searched genes in consumer genetics, and for good reason: it sits at the center of methylation, a process your body uses to build DNA, regulate genes, process homocysteine, and support neurotransmitter balance. Common MTHFR variants change how efficiently that machinery runs. Knowing your MTHFR status is not a diagnosis — but it is a useful window into a pathway that touches many systems.

What MTHFR Does

MTHFR stands for methylenetetrahydrofolate reductase, an enzyme that converts folate (vitamin B9) into its active form, 5-methyltetrahydrofolate. That active folate donates methyl groups needed to convert homocysteine into methionine — a step central to the methylation cycle.

When MTHFR works efficiently, homocysteine stays in a healthy range and methylation reactions proceed normally. When enzyme activity is reduced, folate processing slows and homocysteine can accumulate. Because methylation supports DNA synthesis, gene regulation, detoxification, and neurotransmitter production, a bottleneck here can ripple outward.

The Two Common Variants: C677T and A1298C

Two MTHFR variants are studied most often. C677T (rs1801133) is the more functionally significant: one copy of the T allele reduces enzyme activity by roughly a third, and two copies can cut it by more than half, especially when folate intake is low. A1298C (rs1801131) has a milder effect on activity on its own.

Genotype combinations matter. Carrying variants at both positions can compound the effect. Still, having a reduced-activity genotype is common in the general population and does not, by itself, mean anything is wrong — many people with these variants have entirely normal homocysteine levels.

Health Issues Associated With MTHFR Variants

Research has connected reduced MTHFR activity, primarily through elevated homocysteine, to several areas of interest. Elevated homocysteine is a marker studied in cardiovascular health. MTHFR variants have also been examined in the context of pregnancy and neural-tube development, which is part of why folate is emphasized before and during pregnancy. Some studies have explored links to mood and cognition through the methylation-neurotransmitter connection.

These are associations, not certainties. An MTHFR variant shifts a probability within a pathway; it does not determine an outcome, and it does not diagnose any condition. Diet, B-vitamin status, lifestyle, and other genes all shape where you actually land.

What “Management” Really Means

There is no treatment for a genotype — your DNA does not change. What can change is the input the pathway receives. Adequate folate and related B vitamins (B6 and B12) support the methylation cycle, which is why providers often check homocysteine and dietary intake in people with MTHFR variants. Leafy greens, legumes, and folate-rich foods matter here.
