HLA-B27 is one of the most recognized genetic markers in rheumatology. It is a specific variant of the HLA-B gene, part of the human leukocyte antigen system that helps the immune system tell “self” from “foreign.” Carrying HLA-B27 raises the likelihood of certain autoimmune and inflammatory conditions — though most people who carry it never develop any disease at all.

What HLA-B27 is

The HLA-B gene encodes a major histocompatibility complex (MHC) class I protein that sits on the surface of nearly every cell and presents small protein fragments to immune cells. HLA-B27 is one of many possible versions of this gene. Its job is normal immune surveillance; the trouble arises when, in a susceptible person, this presentation process appears to misfire and drive inflammation against the body’s own tissues.
