Lichen planus is a chronic inflammatory condition that can affect the skin, mouth, nails, scalp, and genital tissue. On the skin it often appears as itchy, flat-topped, purplish bumps; in the mouth it can show up as lacy white patches. It is not contagious and not cancerous, but it can be persistent and uncomfortable. Researchers increasingly see it as an immune-driven condition with a genetic component.

What Lichen Planus Is

At its core, lichen planus is a T-cell-mediated reaction in which the immune system targets the basal layer of the skin or mucous membranes. The result is the characteristic inflammation, thinning, and pigment change. It most often appears between ages 30 and 60 and can flare and settle over months or years. Because several everyday triggers overlap with it, pinning down a single cause is often difficult.

The lesions form when cytotoxic T lymphocytes accumulate at the dermal-epidermal junction and drive keratinocytes toward programmed cell death. These associations do not mean a gene "causes" lichen planus; they describe an inherited tendency toward the immune pattern that underlies it.

Lichen planus has documented associations with hepatitis C infection, certain medications, dental restorative materials, and stress. The hepatitis C link is one of the most studied and is thought to reflect shared immune activation.
